Authorized TGH physicians, leaders and team members are invited to register for SGCS Collaborative High Reliability™ courses using the form below. Listed below are group registration codes for team members, leaders and Reliability Management Team (RMT) members. Please enter the group code that applies to your organizational role into the form below, along with your name, work email address and a password of your choice. The group registration code you use will automatically enroll you in the course(s) you have been assigned.

Distribution of these materials, including but not limited to the SGCS Reliability Response Guide™, is restricted to TGH employees and contractors who need access to the materials under the terms of the TGH-SGCS agreement.
